Children’s Community Nursing Team

We are a team of children's nurses with extensive community training and experience.

Our areas of expertise include chronic disease specialists (Epilepsy, Asthma, Diabetes, CF), Continence Team, Palliative care Team and Home Ventilation/Continuing Care Team.

We support GPs with the acutely unwell child by offering home support, observation monitoring.

Generic Support - early discharge support following planned or unplanned hospital admission.

Drop-in clinics in health centres across Manchester - constipation, eczema, asthma, minor illness, phlebotomy.

Chronic Disease Specialists e.g. epilepsy, asthma, diabetes, Cystic Fibrosis.

Palliative Care Team

Home Ventilation and Continuing Care Team

Work closely with the disabled children's team (social services).

Location:

The team are based at Longsight Health Centre. Clinics are run
at various health centres across the city.

Hours:

8.30 am - 10.00 pm, 7 days a week. This service will offer guaranteed nursing triage within 2 hours of receipt of referral (within office hours).

Referrals:

Referrals are accepted by telephone (24 hour answerphone), fax or post.

Referrals are accepted from:

  • Walk in centres, A&E, O&A units
  • Lead Professionals of Multi-Agency teams
  • GPs
  • Colleagues, fellow professionals, voluntary, statutory and non-statutory organisations
  • Individuals and families known to the team

Hospital and Community Paediatricians

 

We will see any child or young adult under the age of 16yrs (25yrs if they have complex/special needs)
