Children & Young People’s Complex Care Team
We provide care for children and young people who have
underlying medical conditions and require care teams and nursing
support in their own homes.
The team is based at Charlestown Health Centre, but care teams work in the homes of patients across the city.
The Children & Young People’s Complex care Team is a 24 hour service. Office Hours are 8:30 am – 4:30 pm Monday to Friday.

The service is adaptable and care is provided for children and
young people with a range of conditions including ventilation, and
We work with each child/young person and family to provide
individualised care packages to a high standard. The aim of the
team is to maintain normal family relationships within the home for
and for the child/young person to remain safely cared for in their
own home setting, and to minimise the need for hospital admissions
by prompt intervention of the care team.
The service, in liaison with the referring hospital,
co-ordinates complex care packages including identifying the
clinical needs of the children/young people and their families.
With the correct support they will be able to participate in family
and community activities, lead ordinary lives and enjoy their
Referrals are made to the service from a wide range of services
and health professionals, including Royal Manchester Children's
Hospital and other local hospitals.
Referrals should be directed to Soraya Begum, Team Leader, and
