Homeless Families Health Visiting Team

Bed based service:

Intermediate Care bed base service is a service aimed at meeting the needs of people who are not quite ill enough to go into hospital, but not quite well enough to manage on their own at home. This service provides short term rehabilitation to enable service users to regain their optimal levels of independence.

If you are unable to stay at home or to go straight home from hospital, you will be assessed to go to an intermediate care residential or nursing unit, where the team will start working with you. The units have single rooms and support staff are available 24 hours a day.

Community locality teams

If you are able to stay at home, or to go straight home from hospital, you can receive intermediate care in your own home.  The intermediate Care Team will visit you to help you regain your independence.  Carers may also be arranged to help you short term over this period.

While needing intermediate care rehabilitation you will be allocated a Key Worker who will be your main contact in the service. They will act as your advocate throughout your journey within the service and will coordinate the care you receive.


Referrals are accepted from a Health or Social Care worker via a Contact Assessment which can be faxed to 0161 230 3019 then followed by a telephone conversation with the Duty Team Leader on 07790 809582 (bed base) / 07791 574469 (locality teams).

The bed base service provides 24 hour cover, 7 days a week to service users. according to need. Referrals can be made between 8.30 am - 5.00 pm 7days or via rapid response outside of these hours.

The community  locality  teams work from 08:30 until 17:00 7 days a week with an assessment service provided up until 19.30.

The community Intermediate care team are based in the locality teams and can be accessed via the duty mobile 07791574469 or Gorton office 0161 227 3038 who may then refer to correct hub number or arrange for return call.


The Intermediate Care Team consists of Physiotherapists, Occupational Therapists, Nurses, Advanced Nurse Practitioners, General Practitioners, Consultant Geriatricians, Assistant Practitioners and Rehabilitation Assistants. Working with you, the team will aim to achieve the goals you identify to help you become as independent as possible.

Additional Information:

There are many areas that we are proud of within Intermediate care and we have worked tirelessly to improve the standards of care within the service. We have directed our improvements to initially looking at incidents and risks within the service and look at how we can make changes to improve patient outcomes. As a team we have successfully completed the Community safe Project facilitated by AQUA where we were able to improve services for our elderly frail patients.
