Tissue Viability (Citywide)



TV Referrals

Referrals should include at least one patient identification number (hospital number, case note number, NHS number or district number) and may be rejected if this information is not included.

Tissue Viability will see any wounds from groin upwards, and validation of all grade 3 and above pressure ulcers

Central Site: Referrals should be made through CWS (Clinician Workstation).

Trafford and Community: The Tissue viability referral form (on the right hand side of this page) should be completed and emailed to tissue.viability@cmft.nhs.uk or click here to download.


Podiatry Referrals - Acute

All foot wound go to high risk foot team via CWS

- The Tissue Viability team will still validate any pressure ulcers on feet


Podiatry Referrals - Community

Podiatry referrals must be made to Rusholme Health Centre using form - Application for a Podiatry assessment (on the right hand side of this page) or click here to download.


Leg Ulcers - Acute including Trafford

For any wound ankle to groin, referral must be made using CWS to Vascular Nurse Specialist


Leg Ulcers - Community

District Nurse to request GP to refer to Vascular Consultant
