Obtaining Patient & Carer User Feedback for
Most learners are required to obtain patient and carer feedback
on the care they have given. Please click here to download a handy leaflet to guide
you through the process.
Learners have an important role in raising concerns about the
standard of care and any other concerns they have in practice. As a
placement educator/mentor you are the student's first line of
support in practice and you must give them the confidence and
opportunity to speak up. Please sse guidance below for placement

The Mentorship in Nursing and Midwifery Policy was revised
in May 2014.
The main changes surround the annual declaration/ triennial
It is essential all mentors and educators are familiar
with the contents of the policy and are aware of
their accountability and responsibilites in order to ensure they
achieve 100% compliance and are deemed "ACTIVE" on the Mentor
To achieve 100% compliance with the policy, mentors and
educators should attend a mentor update annually and complete an
annual declaration form ( to comply with the NMC Triennial Review
It is imperative mentors are "ACTIVE" on the Mentor Register and
that they are 100% compliant with the policy if they are mentoring

Following the launch of the national Nursing and Midwifery
Strategy - the 6C's in 2012. CMFT has developed a
Nursing and Midwifery Strategy setting out the Values and
Commitments of Nursing and Midwifery staff across the Trust to
provide high quality patient centred care delivered with care and
The Values and Behaviours outlined in this strategy are integral
to patient centred care and should not be isolated to nursing and
midwifery staff. It is important that all staff and students
across the Trust demonstrate these Values and Behaviours.

The Charter demonstrates the placement's commitment to providing
a safe and high quality learning environment for all health and
social care learners to prepare them for future roles working
collaboratively in multiprofessional teams.

The PEF Team organises a programme of
Interprofessional student seminars which are available to all
pre-registration students on placement here in the Trust.

Placement evaluation is important in ensuring that students from
across the disciplines are experiencing high quality learning
opportunities. The aim of the evaluation is to measure the
student's satisfaction and use as a basis to share good practice
and action plan for where improvements are required.
