Part of the remit for the Hospital Transfusion Team is the investigation, analysis and action planning of reported clinical incidents to ensure higher standards of patient safety. This is achieved with the expert input of the divisional Clinical Governance Manager and Trust Risk Management Team. Action plans and trend analysis undertaken by the team is disseminate across the Trust via divisions to clinical areas in an ethos of being open and sharing from lessons learnt. The Hospital Transfusion Team annually participates in the Trust CARM (Clinical Audit and Risk Management) fair as part of our aim to enhance this open and transparent safety culture.
